Noot 30. Thermotherapie

Uitgangsvraag
Wordt thermotherapie (warmte- of koudetherapie) aanbevolen bij patiënten met heup- en/of knieartrose ter verbetering van fysiek functioneren?
Deze uitgangsvraag is beantwoord door middel van een systematisch literatuuronderzoek. Er konden geen relevante RCT’s worden geïncludeerd.

 

Conclusie vanuit de literatuurstudie

Uitkomstmaat ‘fysiek functioneren’ (door de patiënt gerapporteerde uitkomsten): Het effect van thermotherapie (warmte- of koudetherapie) op fysiek functioneren bij mensen met heup- en/of knieartrose is onbekend.

 

Van bewijs naar aanbeveling

Er is geen bewijs voor een effect van thermotherapie in het verbeteren van fysiek functioneren Op basis van het ontbreken van literatuur en de verwachte te verwaarlozen meerwaarde van de interventie boven de gebruikelijke zorg (i.e., oefentherapie en voorlichting/adviezen) is de werkgroep van mening dat de interventie sterk afgeraden kan worden, voor zowel heup- als knieartrose (‘bied interventie niet aan’).

Bied geen thermotherapie aan bij patiënten met heup- en/of knieartrose ter verbetering van het fysiek functioneren.

Zoekstrategie

Er is door het KNGF op 14 augustus 2017 een zoekactie uitgevoerd in PubMed, EMBASE, Web of Science, Cochrane Library, CENTRAL, EmCare en CINAHL, naar samenvattingen van de literatuur (i.e. systematische reviews; SR’s) en RCT’s (RCT’s) met betrekking tot de effectiviteit van thermotherapie op fysiek functioneren bij patiënten met heup- en knieartrose.

Gevonden literatuur

De literatuursearch met betrekking tot niet-oefentherapeutische interventies, waaronder thermotherapie, bij patiënten met heup- en knieartrose leverde 478 SR’s en 1157 RCT’s op. De zoekactie heeft echter geen SR’s of RCT’s opgeleverd die zich specifiek richtten op thermotherapie.

Evidence to decision

Voor het bepalen van de formulering (richting en sterkte) van aanbevelingen zijn, naast de conclusie uit de wetenschappelijke literatuur, ook aanvullende overwegingen (o.a. waarden/voorkeuren van de patiënt, toepasbaarheid in de praktijk) meegenomen. Hiervoor is de ‘Evidence to decision’ methode van GRADE gevolgd en het bestaande ‘GRADE evidence to decision’ formulier in het Nederlands vertaald. Dit formulier is door de werkgroep tijdens een werkgroepvergadering besproken, waarna de formulering van de aanbeveling is bepaald.
