Ongunstige prognostische factoren voor het beloop van (algemene) schouderklachten zijn: langdurig bestaande klachten bij het eerste consult, ernstige pijn, een geleidelijk ontstaan van de klachten, nekpijn, functiestoornissen van de cervicothoracale overgang (CTO), herhaalde bewegingen en ongunstige (repeterende) werkgerelateerde en psychosociale factoren (noot 2 t/m 4).
Glenohumerale instabiliteit en scapulothoracale disfuncties kunnen leiden tot het ontstaan van de klachten, dan wel de klachten in stand houden (lees noot 5 en 6 in de Verantwoording)
Noot 2 Prognostische factoren beloop
Noot 2 Op basis van een systematisch literatuuronderzoek concludeert de NHG-Standaard Schouderklachten (Winters et al., 2008) dat er sterk bewijs is dat 1) klachten die al langere tijd bestaan bij het eerste consult, 2) ernstige pijn en 3) een geleidelijk ontstaan van de klachten voorspellend zijn voor een langduriger beloop van de klachten.
Noot 3 Prognostische factoren herstel
De NHG-Standaard Schouderklachten (Winters et al., 2008) concludeert dat er bewijs is dat bijkomende nekklachten, het uitvoeren van herhaalde bewegingen en de aanwezigheid van ongunstige psychosociale factoren een negatieve invloed hebben op het herstel van schouderklachten.
Noot 4 Prognostische factoren beloop
De NHG-Standaard Schouderklachten (Winters et al., 2008) concludeert dat er bewijs is dat functiestoornissen van de cervicothoracale wervelkolom, werkgerelateerde en psychosociale factoren het beloop bij aanhoudende schouderklachten ongunstig kunnen beïnvloeden.
Noot 5 Associatie subacromiale klachten en glenohumerale instabiliteit
In een aantal studies is een associatie gevonden tussen subacromiale klachten en glenohumerale instabiliteit (Cools et al., 2008). Predisponerende factoren voor instabiliteit zijn: armdominantie, werk / sport / algemene dagelijkse levensverrichtingen (ADL) en kenmerken van hyperlaxiteit (CBO-Richtlijn Luxatie schouder).
Voor het testen van (anterieure) instabiliteit van de schouder kunnen de Apprehension test, de Relocation and release test, de Load and shift test en de Sulcus sign worden gebruikt (Cools et al., 2008; Hegedus et al., 2008; Luime et al., 2004).
NB. Deze tests worden hier slechts genoemd; het betreft geen aanbeveling op basis van het wegen van de beschikbare evidentie over de kwaliteit ervan.
Noot 6 Associatie schouderpathologie en scapulothoracale disfunctie
Noot 6 Een aantal studies toonde een associatie aan tussen schouderpathologie (waaronder impingement) en scapulothoracale disfunctie (Cools & Walraven, 2006). De oorzaak-gevolgrelatie is nog niet bekend.
Vast staat dat scapulaire spierrekruteringspatronen bij patiënten met impingement vaak afwijkend verlopen, en dat deze afwijkingen worden gekenmerkt door een vorm van inhibitie van de m. serratus anterior en de onderste trapeziusbundel. Dit uit zich in scapulaire bewegingsafwijkingen van zowel type 1 (anterieure tilt met prominent zichtbare angulus inferior) als type 2 (te grote interne rotatie of ‘scapula alata’), die gepaard gaan met verkortingen van respectievelijk het posterieure kapsel en de m. pectoralis minor (bij type 1) en van de m. latissimus dorsi (bij type 2) (Kibler et al., 2002).
Bij een scapulaire bewegingsafwijking type 3 staat de scapula in een neerwaartse rotatiestand, waarbij de angulus superior prominent zichtbaar blijft, zowel in rust als tijdens bewegen. Bij type 3 is het musculaire evenwicht tussen de onderste en bovenste trapeziusbundel verstoord en kan de m. levator scapulae verkort zijn.
